Late schedule change appeal form - If you need to add or drop a class after closure of the University Registrar's add/drop period, you will need to complete this form and submit it to CDS 101. If you are adding a class, you will need to complete the Enrollment Change Request which can be found below.
Enrollment change request - This form requires TWO signatures (instructor and department). Complete and submit to CDS 101.
Digital Culture Individualized Instruction/honors thesis form - This form requires TWO signatures (instructor and department). Submit the completed form to the Education Coordinator in Stauffer B268.
Digital Culture override authorization form - This form requires the instructor's signature. Submit the completed form to your advisor or to the Education Coordinator in Stauffer B268.
Digital Culture request for course substitution - The current policy for course substitutions states that students can petition up to three hours per degree category (Digital Culture core, history, and theory, concentration) for up to a maximum of 9 replacement hours. Replacement hours must be related to Digital Culture work and may include courses offered by any unit at ASU. Approved transfer credits from another institution can also be counted as replacement hours. Departmental approval is required. Submit the completed form to the Education Coordinator in Stauffer B243.
